Hi Team Managers!

We apologize for any inconvenience the office being closed may have had on you and submitting your team documents

We kindly ask that the following documents be submitted by November 1st:

  • Jersey Deposit Forms (with posted dated payment) - Applies to U11 and above
  • Covid Waiver Forms

 

Items for sale: 

  • C's and A's - $5.00 each (Full Set $30.00)
  • CMHA Toques - $15.00
  • CMHA Lanyards - $5.00
  • CMHA Window Decal - $5.00
  • CMHA Water Bottle - $7.00

 

The Office will be temporary open at Sardis Sports Complex on the following days:

  • Friday, October 30th from 7-9pm
  • Saturday, October 31st from 9am-noon

Please note the doors will be locked and only one person allowed in the building at a time.
