Jersey Deposit Credit Card Authorization Form

THIS APPLIES TO ALL U9 TEAMS!!!

Chilliwack Minor Hockey Association’s U9 jersey deposit amounts are as follows:

• U9 $50.00
• Jerseys will need be returned to CMHA at the end of each season

1. Team Managers are requested to complete the Jersey Deposit Tracking form as follows:
   1. Complete form as deposits are received from parents.
   2. Collect deposits in the form of cheques, credit card authorization or cash for all jerseys.
   3. Submit form and deposits to the office prior to October 1^st, 2025. Contact Sara Pudwell at info@chilliwackminorhockey.com to arrange an appointment prior to the deadline.
   4. Any team that has not submitted their Jersey Deposit Tracking form and deposits by November 1^st, 2025 will be charged for all jerseys.

Accepted Deposit Types:

Cheques MUST be:

1. Made payable to: CMHA
2. Memo: Player’s name and jersey #
   1. Postdated: March 31st, 2026

Please Note: Any cheque that does not follow the above guidelines will not be accepted.

Preauthorized Credit Card Form:

1. Attached

Cash:

1. Accepted in envelope with jersey #, player name & parent name.

NOTE: All players MUST have a Jersey Deposit on file
